NDIS Funded Accommodation Services

Collective Hope Community Services are committed to supporting our participants to establish a stable and empowering living situation so that they can achieve their goals.

In alignment with this commitment, we offer a range of accommodation services, tailoring supports to enable our participants to fully engage in their local communities.

We offer:

  • Supported Independent Living (SIL)
    SIL is provided to participants with higher support needs who require supports throughout the day, including overnight support.
  • Individual Living Options (ILO)
    ILO support is given to participants who do not require overnight support and can be effectively supported with drop-in supports during the day.
  • Individualised Core Supports (ICS)
    For those who do not have SIL or ILO funding in their NDIS plan, but do require accommodation, we are able to assist with co-designing suitable accommodation supports from core funding.

Supported Independent Living (SIL)

Collective Hope Community Services provides Supported Independent Living (SIL) to eligible NDIS participants with psychosocial disabilities. Collective Hope works with participants with complex daily, behavioural, and functional needs. We work closely with the Department of Justice, leading psychiatric hospitals in Perth, Disability Justice Services, and Homelessness programmes to provide accommodation and tailored support services to enable our participants with higher support needs to live as independently as possible.

Supported Independent Living services are provided to participants who require supports 24 hours a day, including overnight. Their daily supports include 24-hour access to support workers.

These supports may include access to support workers who assist with activities of daily living, ie food preparation, managing and attending appointments, weekly shopping and household tasks.  It may also include assistance with managing and administering medications; systems navigation ie Centrelink, NDIS, and medical reviews.

Our support workers are all qualified in mental health and have experience working with complex participants. We also provide ongoing training to our workers to best work with our participants. Our accommodation sites conduct regular social activities to engage our participants and help them live their lives to the fullest.  We organise parties to celebrate birthdays, holidays, and significant milestones. We also organise and support regular recreational group activities such as going to the beach, and picnics in the park, creating a sense of community and belonging among participants.

Individualised Living Options (ILO)

What does ILO mean?

Individual Living Options provides choice for the participant as they consider their needs and preferences in the design of a flexible package of supports. Implementation usually occurs in stages and the package includes a primary support approach with supplementary supports that ensure that goals are being achieved.

Where there are families and other support people engaged, they will be included with the participant’s permission in the planning process.  Once established, the living arrangement is closely monitored and fine-tuned.

Available within ILO:

  • In the participant’s home – Support is provided full-time or part-time.
  • Host Arrangements – The participant resides full-time in the home of a non-related host who can provide ongoing support.
  • Living Alone – Support is provided in the home of the participant in a variety of ways.
  • Living Together – The participant lives with other people of their choice and receives support.

Daily Living Support

We enjoy discovering where our participants prefer to get their local coffee, the movies they love, and all the other stuff they love about life in our great city.

We then tailor supports, which enables our participants to fully engage in their everyday activities, empowering them to contribute to their local communities.

Collective Hope provides personal care and assistance in daily living for people with disabilities to help them live the life they choose.

The team’s focus is on consistency, continuity and tailoring solutions that meet the individual’s needs.

Our staff are highly skilled and can provide reliable, innovative and flexible support to suit individual needs and goals.

We assist participants to develop daily living skills and provide support with a variety of tasks which include:

  • Personal Care
  • Grocery shopping
  • Meal planning and preparation
  • Assistance with medical needs
  • Healthy lifestyle support
  • Travel training.
