Positive Behaviour Support, Support Coordination, and Recovery Coaching

Collective Hope provide allied health services to participants who require Positive Behaviour Support, Recovery Coaching, and Support Coordination services. We are a registered provider with a highly qualified team including psychologists, social workers, and counsellors. Positive Behaviour Support

Positive Behaviour Support (PBS) is a person-centred approach of providing support to manage and overcome challenging behaviours and improve the overall quality of life of a participant. The PBS framework is based on key values that are committed to providing support which include:

  • Increase choice
  • Promote equality and human rights

Collective Hope PBS specialises in complex mental health and psychosocial disabilities such as Autism, ADHD, Schizophrenia, Bi-Polar Affective Disorder, Anti-social behaviour, and Intellectual disabilities in participants across all age groups, including children and young people. Our Positive Behaviour Support specialists will work closely with the participants to develop a plan that will help with any challenging behaviours that they would like to work on.

Our practitioners can provide:

  • Functional behaviour assessments
  • Interim and comprehensive behaviour support plans
  • Reducing restrictive practices
  • Positive behaviour training
  • Individual skill building sessions

Support Coordination

Support coordination helps you navigate your plan and supports you through the process of finding the right supports. Support coordination is a capacity building support which helps you to:

  • Understand and use your NDIS plan to pursue your goals.
  • Connect you with NDIS providers, community, mainstream and other government services.
  • Build your confidence and skills to use and coordinate your supports.
  • A support coordinator or specialist support coordinator delivers support coordination services.

There are three levels of support coordination that can be included in your plan:

  • Support connection – This support is to build your ability to connect with informal, community and funded supports enabling you to get the most out of your plan and pursue your goals.
  • Support coordination – coordination of supports: This support will assist you to build the skills you need to understand and use your plan. A support coordinator will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community.
  • Specialist support coordination – This is a higher level of support coordination. It is for people whose situations are more complex and who need specialist support. A specialist Support Coordinator will assist you to manage challenges in your support environment and ensuring consistent delivery of service.

You may also wish to focus on a specific goal in your NDIS plan, such as those related to finding suitable home and living supports. You can use your support coordination funding to work with a support coordinator who has the skills, knowledge and experience to meet your specific needs.

Recovery Coaching

Our recovery coaches will partner with you on your path towards personal growth and well-being.

Our skilled team of coaches provide individualized support, empowering you to:

  • Set meaningful goals
  • Develop essential life skills
  • Help manage your NDIS supports
  • Navigate challenges.

If yes, please email a copy to our NDIS Consultation team, hello@collectivehope.org.au.

If no, please contact your Support Coordinator and request a copy to be sent to our NDIS Consultation team.

If you do not have a Support Coordinator, you will need to request a copy from the NDIS.

Referral Form

If you have a plan and have a support team in place, please have the Support Coordinator complete our referral form.

Please forward copies of the following (where applicable):

  • Functional Capacity Assessment (FCA)
  • Occupational Therapy Reports (OT)
  • Positive Behavioural Support Plan (PBS)
  • Brief Risk Assessment (BRA)
